Background Information
CHED’s mandated functions under Administrative Order, (AO) No. 56 dated January 24, 2003 created the Philippines Seafarers’ One-Stop Center (PSOC) now Overseas Filipino Workers One-Stop Processing Center.
As per Memorandum from the Executive Office dated April 2, 2007 which was also reiterated through Memorandum from the Executive Director dated January 24, 2011, CHED - Office of Student Services will continue the issuance of CAV of academic records for student's request at the OFRW One-Stop Processing Center, following the new authentication Procedures provided for in Executive Order (EO) No 582 and CHED Memorandum Order(CMO) No. 17, series of 2013 “Enhanced Policies and Guidelines for the Certification, Authentication and Verification (CAV) of School Documents”.
Since 2003, the Philippines Seafarers’ One-Stop Center (PSOC) now Overseas Filipino Workers One-Stop Processing Center (OFW-OSPC) helped and assisted the Overseas Filipino Workers (OFWs) in securing their Certification, Authentication and Verification (CAV) even if they graduated from the provinces outside Metro Manila. Requirements
Submit filled up Application form (available at www.ched.gov.ph (ISAD Form1)), Dring Original Copies of the following documents at the Office of Student Development and Services (OSDS) and:
- Two (2) sets of photocopied Transcript of Records (T.O.R)
- Two (2) sets of photocopied Diploma
- Two (2) sets of photocopies Related Learning Experience (RLE) (If applicable)
- Certification from Higher Education Institution (HET), (If Applicable)
If Representative:
Authorization letter duly notarized and photocopy of valid I.D. of both parties
Schedule of the Availability of Service: Monday to Friday 8:00 am to 5:00 pm
